Reporting Requirements

If you believe you have experienced an exposure incident, and you have not yet sought evaluation, please do so immediately. The information below is not time-sensitive, but caring for your heath is! Get evaluated. Then come back and read this important information about University reporting requirements.

Why?

MSU policy mandates reporting an exposure incident. No exception to this policy exists. That means even if you choose not to be treated or do not require treatment, you still must report the incident. This is a necessary policy that improves and makes consistent the response to students who have experienced an exposure incident, as well as assures the safety of the entire community, you included. What?

To report an incident, you will use the "Student Exposure Report for Tuberculosis, Blood Borne Pathogens and Zoonotic Disease" form. (You will need the free Adobe Acrobat Reader to view and print this report.) These reports are treated confidentially once they are returned. (top)

Who?

You may fill out the report, or have another party (for example, at an off-site location) complete it. But, you and anyone else who adds data to the report must sign it before submitting it. (top)

When?

We ask that you submit the completed report within 10 working days of the incident. (top)

How?

Requesting a reporting form is simple: you may download it by clicking on the "form" link above, or you may contact your college administrator. You may also call the Occupational Health Service at 517-353-9137 to request a form. The people who give you the form can also answer any questions you may have about how to fill it out. (top)
