Immunization Self-Report
Immunization Guidelines
Updated: 10/3/07
MSU distributes materials related to vaccinations in the college-age population to our students as part of a proactive public health campaign. These efforts are designed to prompt students and their families to review immunization records to ensure that students are protected against preventable contagious diseases, and to bring their immunization status up to date if it is not current. Completing the required Immunization Self-Report form provides the University with confidential data that facilitates the University's response during a contagious disease outbreak as well as facilitates provision of routine healthcare on campus.
You may review the email to new students, which explains the University's recommendations on four important vaccines: hepatitis B, varicella (chickenpox), tetanus/diphtheria/pertussis, and meningococcus. The letter introduces the University's policy on the required completion of the Immunization Self-Report form for new undergraduates, and gives information regarding who to contact for assistance.
Information about the recommended vaccinations and their costs are described in the Immunization Guideline Sheet. A brief set of Frequently Asked Questions anticipates some questions that students and their families may have. Make sure you review these last two documents.
MSU's recommendations are consistent with the most recent recommendations from national health authorities. You may visit their Web sites for more information.
