Immunization Self-Report

Prompted by new recommendations from the Centers for Disease Control and Prevention (CDC), MSU regularly updates its Immunization Policy. All new undergraduate students are asked to review their immunization status and consider getting the recommended vaccines so their immunization status is up-to-date. The University requires all new undergraduate students to complete the Immunization Self-Report form. Failure to complete this requirement will affect your ability to register and enroll. This information is helpful to the University in the delivery of routine health care and in the unlikely event of a disease outbreak.
